Fresource Friday: Homelessness Medical Questionnaire for GPs
This week's free* resource is a form local authority homelessness teams can use to make inquiries of a general practitioner in connection with a homeless applicant together with a covering letter.
How to download
To download the documents you'll need to:
- work for a local authority or housing association in England or Wales;
- create an account (if you haven't already done so) using your work email address and wait for a second confirmation email; and
- go to the relevant pages (questionnaire / covering letter) in the Resources section.
